Endometriosis is a painful condition in which the endometrium (the innermost layer of the uterus) is found outside the uterus. It impacts ovaries, fallopian tubes, and other pelvic organs like ovaries, bowel, bladder, peritoneum etc. It is a disease of reproductive age group and mostly presents in women of 30- 40 years of age.
In approx. 40% of infertile women, the cause is endometriosis. It reduces the number and quality of eggs and also interferes with the motility of fallopian tubes and in extreme cases may even cause blockage of the tubes.
The cause of endometriosis can be retrograde menstruation, weak immune system, endometrial cell transport, transformation of cells that forms the inner lining of the abdomen. As defined above, endometriosis is presence of endometrial tissue outside the uterine cavity. So, the endometriotic implants respond to hormonal changes during menstrual cycle in the same way as does the endometrium. They grow and bleed during the menstrual cycle which causes irritation and inflammation of the surrounding tissue which further leads to scarring and adhesion formation with the adjoining tissues. This inflammatory process occurs and worsens in every cycle and is the reason of severe dysmenorrhea before and during menstruation.
Dr.Shweta recommends pelvic examination, , ultrasound, MRI, and laproscopy(gold standard) for proper diagnosis.
Endometriosis can be treated but not cured as it is a recurrent disease. Medical management is used to get relief from pain but is not definitive. Even after surgical excision of the implants there is a chance for relapse. Relapse is seen in 40–80% of women within 2 years of surgery and is linked to the severity of the disease. Medical management combined with surgical may help extend the symptom free interval.
The most conservative therapy for endometriosis is with medications. Non-steroidal anti-inflammatory medications, like ibuprofen, may help with the pain associated with endometriosis. Some doctors may prescribe medications that affect a woman’s hormones in order to help with endometriosis pain. Some examples are oral contraceptive pills and gonadotropin releasing hormone (GnRH) agonists, oral progestins (dienogest)
Surgery can be used to remove the endometriosis or to burn the endometriosis lesions outside of the uterus to make them go away. It is also used to get rid of scar tissue so that the ovaries and tubes can go back to their normal location in the pelvis. Surgery has been shown to improve pain symptoms associated with endometriosis and may also help women become pregnant. If a woman with endometriosis is not interested in becoming pregnant, she and her doctor may decide to remove the ovaries and possibly the uterus If a woman with endometriosis is having trouble getting pregnant, there are different medications and treatments available that can help her to become pregnant. Excision of endometriosis implants and endometrioma of ovary; opening up of blocked tubes; clipping of hydrosalpinx are few of the many surgical procedures that can be offered to infertile couples with good outcomes both in natural cycles and IVF.
